Program Overview

Managing population health is the next frontier for healthcare providers- but after decades of treating illness with episodic care, how can physicians, nurses, and their care teams make the shift and implement this wholesale change in care delivery? This unique program, designed by clinicians and nurses who are experts in population health, will provide a comprehensive framework of the steps needed and the key issues for provider organizations to consider as they re-design care coordination within their practices. Participants will leave the program with specific knowledge of the new care delivery environment, preventive and interventional strategies for managing health, and next steps for their organizations.
