Clinical Health Coach Fusion Training has emerged from the experience of training hundreds of healthcare professionals across the country. The goal is to provide the flexibility and valuable standalone elements of the Clinical Health Coach Training Online, with an engaging onsite, Two-Day Live Intensive training that features the most highly rated elements from our flagship Clinical Health Coach Training onsite.

Fusion Training Begins Online (enrolling through April 14th). A personalized, convenient, 26 hour self-paced online learning experience with live weekly teleconference practice sessions can be completed in as little as 6 weeks. It is recommended participants complete the majority of the online modules prior to the Two-Day Live Intensive.

Two-Day Live Intensive (July 12 - 13, 2017) provides enrollees access to leading faculty and delivers additional curriculum previously available only in Clinical Health Coach onsite. Participants will experience dynamic learning, clinical coaching demonstrations, live practice and networking opportunities.

What is clinical health coaching?
Clinical health coaching is first and foremost, a very particular set of skills and attitudes enabling a wide range of healthcare professionals to partner with patients to achieve health outcomes that matter. Since most healthcare actually takes place outside the provider setting, this training is designed to help the care giver align the care processes while activating the patient to build self-care skills, improve health behaviors and inspire their accountability.

More about the faculty
Iowa Chronic Care Consortium (ICCC) was formed as a non-profit in 2002 with the vision of reducing the burden of chronic conditions in populations through collaborations with health systems, health plans and primary care practices. Validated results from ICCC demonstrations
with health systems and Medicaid beneficiaries, in Iowa and other states, affirmed the unique value of coaching as primary a component strategy of its population health approach.

Credits are available through the Interstate Postgraduate
Medical Association for nurses and for most other healthcare professionals based upon AMA PRA Category I Credits™. Typically this training provides nursing CEU’s at 31.2 hours and AMA PRA Category I hours at 26.1. Full information on CBME/CEU credits and disclosures are available at www.clinicalhealthcoach.com

Eight (8.0) hours of Qualified Education credits will be granted for this program toward advancement or
recertification in the American College of Healthcare Executives



Full refund available minus a $50 administrative fee through April 14th. Cancellations or substitutions will be accepted until April 14th or until the student begins online training. Once online training has begun, no refunds or substitutions will be available.
