SUMMARY:The West Health Accelerator: Transforming The Care of Older Adults in Hospitals
DESCRIPTION:This event has concluded. \n\n\n\n\n\n\n\n\n\n\n\nTransforming hospital care for older adults is imperative\, and while there are clinical solutions to target the needs of this population\, many hospitals have not operationalized these solutions across units or beyond pilot programs. Hospitals are actively working to reduce lengths of stay and costs\, while simultaneously addressing the needs of increasingly complex patients. The opportunity to spread these learnings and employ them at hospitals across Mass General Brigham is enabling better care for older adults through proven solutions. It assesses hospitals on their commitment to providing age-friendly care\, focusing on key aspects such as patient goals\, medication management\, frailty screening\, social vulnerability\, and age-friendly leadership commitment. Hospitals must demonstrate compliance with these areas to receive credit under the measure.  \n\n\n\n\n\n\n\nThis webinar will focus on the Lahey Hospital & Medical Center’s commitment to improving care for older adults while meeting these requirements. This includes leveraging the EHR and practices already in place while creating new processes to address these age-friendly principles and ultimately improve outcomes for this population. Participants will gain information on best practices aimed at achieving these goals and meeting the CMS requirements.
