SUMMARY:Addressing the Capacity Crisis Through Cross-Continuum Collaboration
DESCRIPTION:This event has concluded. \n\n\n\n\n\n\nHealth systems nationwide are continuing to struggle with capacity issues. Patients are likely to spend more time in the ED and patients on inpatient floors who need the next level of care are facing discharge delays due to bed shortages in post-acute facilities. These trends are intertwined with ongoing labor shortages\, particularly in the skilled nursing facilities and home health settings. Given this perfect storm of challenges\, it’s more important than ever for hospitals and post-acute providers to prioritize collaboration across care settings. In partnership with the Massachusetts Health & Hospital Association\, PointClickCare is featuring a panel discussion on the role of acute and post-acute collaboration in the context of the capacity crisis. Healthcare thought leaders will share their experiences with these challenges and strategies to overcome them. \n\n\n\n\n\n\n\n\n\nHighlights include:• How to build and enhance relationships within your network• Opportunities to leverage technology to support more effective care coordination• The effect on length of stay\, readmissions\, and overall ability to serve patients \n\n\n\n\n\n\n\nEvent Panelists:  \n\n\n\n\nPatricia Buiocchi\, BSN\, MBA | Sr Vice President of Post Acute Care Services and Southcoast Health at Home\, Southcoast Health System \n\n\n\nChristina Della Croce\, MBA\, OT I  Executive Director\, Acute and Post-Acute Network Performance\, Atrius Health \n\n\n\nAlessio Miniello\, R.N.I  Chief Clinical Officer\, Sherril House  \n\n\n\nAdam Delmolino I Senior Director\, Virtual Care & Clinical Affairs\, Massachusetts Health & Hospital Association \n\n\n\nCathy Guttman I  Principal Customer Success Manager Post-Acute Care Network Management\, PointClickCare \n\n\n\nRachel Burnard I  Senior Clinical Solutions Lead\,  PointClickCare

SUMMARY:Social Prescriptions: Addressing Behavioral Health through Arts & Culture
DESCRIPTION:THIS EVENT HAS CONCLUDED. \n\n\n\n\n\nMassachusetts was one of the first states in the nation to practice “social prescribing” – in which care professionals refer patients to non-clinical services to support their wellbeing. In January 2020\, the Mass Cultural Council launched a pilot in which eight cultural organizations were chosen to participate in CultureRx\, designed to positively affect the health and well-being of the people of Massachusetts through participation with cultural organizations. The pilot’s intent was to demonstrate the protective and therapeutic effects of arts and culture. \n\n\n\n\n\n\n\n\n\n\n\n\n\nThis webinar will provide an overview of the pilot: implications of the program\, and considerations for future opportunities to expand the practice of social prescribing.
