It assesses hospitals on their commitment to providing age-friendly care\, focusing on key aspects such as patient goals\, medication management\, frailty screening\, social vulnerability\, and age-friendly leadership commitment. Hospitals must demonstrate compliance with these areas to receive credit under the measure.  \n\n\n\n\n\n\n\nThis webinar will focus on the Lahey Hospital & Medical Center’s commitment to improving care for older adults while meeting these requirements. This includes leveraging the EHR and practices already in place while creating new processes to address these age-friendly principles and ultimately improve outcomes for this population.
