It is crucial that patients receive the right care, at the right time, and in the right place after leaving the hospital, so that their individual goals and needs are met and their quality of life is as optimal as possible.
MHA’s Continuum of Care Council undertook a collaborative process to create an “After the Hospital” post-acute care guide for patients, families, providers, and clinicians. This guide is the first of its kind among healthcare systems across the country.
